ALL CHILDREN MUST WEAR A UNIFORM
CHILDREN WITHOUT A UNIFORM WILL BE SENT HOME!
Before your child can be accepted into Peaks Little Angels, Inc. the
following forms must be completed and submitted:
Application for Day Care Services Child Health Appraisal
Emergency Contact Parental Consent
Agreement for Payment & Services
YOUR CHILD WILL NEED:
Uniform Book Bag Small Blanket
Painting Smock Toothbrush Change of Clothing
*Children are provided with a well balanced breakfast, lunch and
afternoon snack. Meals are prepared in our modern kitchen facility.
All parents must fill out an Archdiocese of Philadelphia Food Program form to be eligible for Free and Reduced Meals.
ALL CHILDREN MUST BE SIGNED IN AND OUT DAILY!
